Fixed $httpd:complete_url proxy header detection for Caddy

$httpd:complete_url was checking for lowercase proxy header keys ("x-forwarded-host", "x-forwarded-proto") but Caddy sends them capitalized ("X-Forwarded-Host", "X-Forwarded-Proto"). Updated to check both cases and handle WAIF request objects in addition to MAP.
